Please provide the following information:

• Inform your doctor if you have had an allergic reaction to contrast dye (X-ray or IVP) or iodine.
• Complete any necessary tests required for your procedure as prescribed by your doctor. This includes all blood work.
• Obtain your prior imaging studies. Your doctor’s office can assist you. You must bring these on the day of your procedure.
• Please have your insurance card ready when you call and upon your arrival.
